I don't have one at the moment but intend to create a simple Word document similar to this:

Are you looking to use the journal on the computer or by hand writing into it. There are a number of options that are pretty reasonable to have pretty nifty covers put on a template like was just demonstrated if you wanted to carry it with you. If you are looking to use your computer, there are some very simple ways to implement your journal as well.
